The working hypothesis is that primary health care teams with effective teamwork activate empanelled MCO enrollees (who subsequently activate family members) to engage in health promoting activities, including participation in worksite wellness programs and MCO health education classes. As a consequence, these enrollees and their family members have better health outcomes than enrollees (and family members) empanelled to primary health care teams that are less effective at activating their patients to engage in health promoting activities. Organizational culture (perceived practice design and efficiency, morale and satisfaction) has been measured by survey in mid-2000 (80% response rate) and 2002 (91% response rate).
